Here at Utah Therapeutic Health Center, we provide patients with thorough medical evaluations of their qualifying condition. After meeting with one of our qualified medical providers, we'll walk you through the entire EVS application process on-site.

We cannot accept insurance. The cost of the visit is $250 payable by cash, card, or HSA.

We know the UDOH application process can occasionally be frustrating so we have an EVS specialist on staff to help with any snags and issues you may encounter while applying.

Please park in the visitor parking lot in the back of our building. We are on the south side of the street, sandwiched between the Arctic Circle on the corner of 700 E and 3900 S and an apartment complex.
