Introduction to Sacraments ACMPR forms are still accepted and valid as of March 21st, 2014 Sacraments healing & education centre foundation, our goal is to provide patients a a medical cannabis source for consultations, compassionate support, counselling and access to safe medical lab testing products. Sacraments operates by walk in or appointment and you may call or email the office to set up a consultation appointment that works best for your self. During the consultation we discuss what your goals are and which products may work best to meet your needs & educate you with references from the most updated medical cannabis information. Sacraments does not offer physician referrals and there are no doctors on site. If you do not have a doctor to sign the forms, you may visit a local walk-in clinic with the referral letter from sacraments. All membership information is held with the utmost attention towards confidentiality & respect of confidentiality. All information provided will be held to the strictest level of privacy by the management and staff of Sacraments. Membership information is not open to scrutiny by other members, organizations or government representatives. We request that our members also respect the following rules, regulations & guidelines of Sacraments. • No use of medical cannabis on the premises. • No use of foul/abusive language or behaviour towards staff or other members • Respect the privacy of other members • Respect the surrounding businesses and community • We reserve the right to refuse service.
