The process to book and complete an appointment: (step by step)

1)Book your appointment using the online schedule on our website. Enter the coupon below the calendar.

2)Intake form submission: Complete forms in your patient dashboard.

3) At the time of your appointment, click the link in your email.

5) Expect to see your patient recommendation in your patient profile the same day.

For those who select full-service, within 24 hours of your appointment an Elevate Holistics team member will reach out via text or email about confirming your email with the OMMA. Once that is done, we will upload all necessary information to the application and inform you when it is time to log in to submit the application. You are required to log in using the email and password combination you've given us to complete the transaction. We're willing to take payment over the phone if that makes things easier. This means that we'll log in and you will tell us your CC number so we can submit the application.
