The process to book and complete an appointment: (step by step)

1)Book your appointment using the online schedule. If you want us to manage the application process to the state on your behalf, select the full-service add-on.

2)Intake form submission: Complete the forms in your patient profile

For those who choose the full-service option and don't feel comfortable uploading pictures to the form directly, feel free to send us the pictures via text.

3) At the time of your appointment, either go into CannaBelews storefront to visit with the doctor over the computer or go into your email and click the link on your phone.

4) Sit tight and wait for the doctor to start the video. 

5) Expect to see your patient certification in your profile same day.

For those who select full-service, within 24 hours of your appointment an Elevate Holistics team member will reach out via text or email about confirming your email with the DHSS. Once that is done, we will upload all necessary information to the application and inform you when it is time to log in to submit the application. You are required to log in using the email and password combination you've given us to complete the transaction. We're willing to take payment over the phone if that makes things easier. This means that we'll log in and you will tell us your CC number so we can submit the application.

About Us

This is a partner location with CannaBelew's. You can go to the store for your appointment if you so choose. 

We live out our values by providing a solely telemedicine operation for medical cannabis evaluations. With a team offering assistance with the application and helping people who don't feel comfortable using technology, we're able to see people same day, and submit DHSS compliant medical marijuana applications to the state within 24-hours. Everyone sees our medical cannabis certified doctors virtually either from the comfort of your own home or at a location designated on the site over an iPad.

We do not require medical records to have an appointment. Each appointment lasts about 10-minutes.
