How to Book An Appointment:

2. Select the appropriate appointment type:
3. Complete the necessary patient registration forms to provide Duber Medical the necessary information for the doctor to review
4. Click Check-out. Once your appointment is confirmed, you will receive a confirmation email with the appointment date, time and Zoom link to join the video consultation with the doctor. Please ensure you read the Zoom instructions at the bottom of the email to successfully join the meeting.
5. Join the virtual consultation a few minutes ahead of your scheduled appointment. The doctor will let you in when he or she is ready to begin the video chat
6a. For those who qualify for a medical marijuana card: After the consultation, Duber Medical will register you in the Ohio Medical Marijuana Registry database. You will get an email from the state of Ohio within an hour of your appointment, asking you to activate your card. You will need to create credentials and login to the Ohio Medical Marijuana database. Once logged in, you will pay the state activation fee and an electronic copy of your card will appear in your profile.
6b. For those who do not qualify for a medical marijuana card: Duber Medical will issue you a refund within 24 hours of your appointment, minus a small credit card processing fee. This refund can take 5-7 business days to appear to the credit card on file. If you have any questions about your refund, please contact us
7. Please review your medical marijuana card to ensure all personal information is accurate. If so, you are ready to head to the dispensary! If any changes are needed, please contact Duber Medical and we will make them as quickly as possible.

Dr. Kamal N. Morar - A native of Dayton, Ohio. He completed high school at Centerville High School and went on to college at the University of Cincinnati. After medical school, he completed an internship in general surgery followed by a diagnostic radiology residency. He is Board Certified in Diagnostic Radiology. After his residency he completed a fellowship in Vascular Interventional Radiology at the Cleveland Clinic Foundation.
Dr. David Zainey - A board-certified physician and U.S. Air Force veteran, Dr. David A. Zainey was born into a military family at the United States Military Academy West Point in New York. He grew up in Beavercreek, Ohio, while his family was stationed at nearby Wright-Patterson Air Force Base.
Dr. Zainey graduated from Wright State University in Dayton with a bachelor of science in biology and chemistry in 1979, and received his doctor of medicine degree from WSU’s School of Medicine in 1983. Dr. Zainey went into private practice in Springfield, Ohio, specializing in primary care pediatrics – where he remained for nearly 30 years. In 2015, Dr. Zainey began transitioning to the practice of integrative medicine for both adults and children. He has been recommending medical marijuana since 2018.
Dr. Joseph Hoffman - Dr. Hoffman did his undergraduate work in Michigan before going to Medical School in Missouri at Kirksville College of Osteopathic Medicine and Surgery. He credits this education for his ability to understand the patient’s illness by understanding the whole person. From Missouri, Dr. Hoffman went to Boston Massachusetts to do a rotating internship at Massachusetts General Hospital and Massachusetts Osteopathic Hospital. Moving into the educational side of medicine he returned to teach Emergency Medicine at the University of Wisconsin, the University of Texas, and Johns Hopkins School of Medicine where he was involved in stroke and cardiac research.
Dr. Hoffman’s belief that one must treat the entire person to truly create proper results made it an understandable transition to medicinal cannabis as it is often the “hidden” relief many patients have seeking.
