How to Apply: 

1.  Visit our website and select “get started” to fill out our patient paperwork 
2.  Pick a package! Our fee for Evaluation and Recommendation for first timers or renewals is $149. The fee for our white glove service where we do it all for you is $249 ($100 state application fee is not included)
3.  Once you are in your patient portal, upload a picture of your valid Oklahoma drivers license, Oklahoma ID, or US Passport
4.  Connect with one of our online doctors by clicking the “start visit” button
5.  View uploaded documents in your patient portal under “my documents”
6.  Register with OMMA to obtain a patient medical card. Valid for 2 years ($149 package)
7.  Or sit back and wait for your medical card to arrive in the mail ($249 package)
