American Telemed Evaluations, LLC, is a fully-fledged medical provider dually licensed in CT and NY that offers comprehensive medical cannabis evaluations to new and existing patients diagnosed with a qualifying condition in Fairfield County, CT or NY State. Patients will be evaluated by a board-certified Family Nurse Practitioner, Barbara Scorziello, NP with over 37 years of experience specializing in Family Medicine who is state-approved to issue recommendations for medical cannabis treatment. She will carry out each evaluation in a discrete and professional manner, providing a thorough review of each patient's medical condition and history to determine if medical cannabis will be therapeutically beneficial to add to your treatment regimen to improve your quality of life. Fairfield County, Connecticut Residents: A face to face encounter is required for new patient certifications at a mutually agreed upon location. Covering Bridgeport, Cos Cob, Darien, Easton, Fairfield, Greenwich, New Canaan, Norwalk, Old Greenwich, Ridgefield, Riverside, Stamford, Trumbull, Weston, Westport, Wilton.

New York Residents: A telemedicine encounter (via computer or smartphone) will be conducted.

Your medical information will be kept confidential on a HIPPA compliant platform where you will have access to all documents. If you meet the criteria to become a patient, you can purchase your medical cannabis at a local state authorized dispensary.
